IMPORTANT ** THE SHIPPING NAME must be registered with the U.S. Post Office as living at the address your having it shipped to. If new address, or living at a friends house, then use your friends name "care of" your name. Or let the postman know to put your name on the mailbox. 90% of the orders that get returned to me are for this very reason. If your order gets returned to me then you will need to pay the new shipping charges to have it reshipped.

Please include an email address so I can email you your tracking number. I promise, no spam.

If you live in an area where the weather is below freezing, please select the upgraded "warmers" as shipping method. These warmers will keep your spores warm for up to 3 days.

After completing this form, print it or write request neatly, and mail it enclosed with cash or a money order with payment made to T H. Please do not write anything else on the money order but T H. We also DO NOT Accept checks, thank you.
Please DO NOT send us any mail that requires our signature. We will not sign for any mail or packages, no exceptions. Your letter will go unclaimed and returned to you if you ignore this important request. This includes Express mail, regeistered mail etc..You may check the box to "waive the signature" so the postman will drop it off without a signature. You may also put a tracking number on it if you like so you know when it was delivered, but no signatures. We do have important reasons for doing this, thanks for understanding.
